Group Enrollment Form

To enroll 3+ people from the same school or district, please complete the following enrollment form.  This form should include complete information for one individual participating in the event, and the email contact for the person responsible for invoicing/payment.  When you indicate the number of other individuals enrolling, you will be asked to provide their names and contact information.  They will then receive an email with instruction to complete the enrollment process.
